AI charting and SOAP notes for dental practices

TL;DR

An AI dental scribe listens to the operatory conversation and produces a complete, code-ready SOAP, DARE, or narrative note in roughly 45 seconds. It eliminates after-hours charting, standardizes documentation across providers, and drops ODA codes and FDI tooth numbers directly into the chart. Nurvivo Dental is the only Canadian-built scribe with this workflow, hosted on AWS Canada (Toronto) and PHIPA-aligned.

Overview

AI charting replaces the typing dentist or assistant. The scribe captures the clinical conversation, structures it into the note format your practice uses, and pre-fills procedure codes and tooth numbers. The clinician reviews and signs — typically in under a minute per patient.

The problem this solves

Dental documentation is the single biggest time sink outside chairside care. Most dentists spend 60-90 minutes per day on notes, often after hours. Inconsistent formatting, missing codes, and forgotten details create billing gaps and medico-legal exposure. Hiring a human scribe is expensive and hard to staff.

How it works

1. Ambient capture

A microphone in the operatory captures the dentist-patient-assistant conversation. Nothing is stored as raw audio after the note is generated.

2. Structured drafting

The model produces a SOAP, DARE, or narrative note matching your practice template, with ODA codes and FDI tooth numbers inferred from context.

3. Clinician review

The dentist edits any field, accepts the draft, and signs. The note lands in the chart fully formatted in roughly 45 seconds total.

4. Downstream automation

Confirmed codes flow to the treatment plan, insurance pre-authorization, and the patient summary — no double entry.

Benefits

  • Roughly 45-second note generation per patient
  • Consistent formatting across every provider
  • ODA codes and FDI numbering auto-populated
  • No after-hours charting
  • Reduced billing gaps and medico-legal exposure

The Nurvivo approach

Nurvivo Dental produces SOAP, DARE, and narrative notes in the format your practice already uses, with Canadian codes (ODA) and FDI tooth numbering built in. Raw audio is discarded after note generation; only the signed text note is retained. Hosted on AWS Canada (Toronto), PHIPA-aligned, with a zero-training policy.

Clinical workflow: before, during, after

A step-by-step view of what changes — and what doesn't — across a normal patient visit.

Before the visit

  1. Patient consents to AI-assisted documentation as part of standard intake (one-time, plain-language wording you can adapt).
  2. Per-provider note templates (SOAP, DARE, narrative, custom) are configured once.
  3. Operatory microphones are paired and tested. Staff confirm role-based access for who can author vs sign notes.

During the visit

  1. The dentist works normally — there is no on-screen prompt to think about.
  2. The scribe captures the operatory conversation ambiently. Nothing is shown to the patient.
  3. At appointment end, a draft SOAP/DARE/narrative note appears in the chart in ~45 seconds with ODA codes and FDI tooth numbers pre-filled.

After the visit

  1. Dentist reviews and signs the note — every field is editable. The signed note is the source of truth.
  2. Raw audio is immediately discarded; only the signed text note is retained in the chart.
  3. Confirmed codes flow into the treatment plan, insurance pre-authorization, and patient summary.
  4. Every edit, signature, and code change is written to an immutable audit log.

Privacy, consent & data handling

Nurvivo Dental is PHIPA-aligned for ambient clinical documentation. Audio is never persisted, clinical data stays in Canada, and the signed note — authored by the dentist — is the only artifact retained.

Consent

Patient consent for ambient AI documentation is captured at intake using plain-language wording your practice can adapt. Consent is recorded per patient, revocable at any time, and revocation immediately stops new ambient capture for that patient. Every consent event is logged.

Retention

Raw operatory audio is discarded as soon as the draft note is generated — it is never persisted to disk. The signed text note follows your practice's standard dental record retention policy (typically 10 years past last visit per provincial college guidance). Audit logs are retained for the same period.

Data handling

All processing happens on infrastructure hosted in AWS Canada (Toronto). Data does not leave Canadian jurisdiction. Access is role-based with SSO/MFA, encrypted in transit (TLS 1.2+) and at rest (AES-256). Nurvivo Dental signs PHIPA-aligned agreements with each practice and operates a zero-training policy: your clinical conversations and notes are never used to train external or internal AI models without an explicit, separate opt-in.

Frequently asked questions

How long does it take to generate a dental SOAP note with AI?+

Modern AI scribes generate a complete, code-ready note in roughly 30–60 seconds after the appointment ends. Nurvivo Dental targets about 45 seconds for a full SOAP, DARE, or narrative note.

Which practices are eligible to use an AI dental scribe?+

Any licensed dental practice in Canada can use Nurvivo Dental — solo GP, group, specialty, and DSO. There is no minimum chair count, operatory count, or PMS prerequisite. A microphone in the operatory and an internet connection are sufficient.

How accurate are AI-generated dental notes?+

Quality is measured on three axes: clinical completeness, code accuracy, and editing time. Nurvivo Dental targets a draft the clinician signs with under 10% edits in routine restorative, hygiene, and exam visits. Complex surgical and pediatric behavior cases typically require more clinician review.

Does the AI scribe support ODA procedure codes?+

A Canadian-built scribe should. Nurvivo Dental infers ODA codes from the clinical conversation and drops them into the chart and treatment plan automatically. International scribes typically do not handle Canadian code sets — verify this in writing before signing.

Does it work for hygienists and assistants too?+

Yes. Notes can be authored from the hygienist's or assistant's voice with the dentist as the final signer. Per-provider templates keep formatting consistent across the team.

Is patient audio stored?+

Not by Nurvivo Dental. Raw audio is discarded immediately after the draft note is generated; it is never written to disk. Only the signed text note is retained in the chart. Vendor policies vary widely on this — always confirm in writing before signing.

Is the scribe PHIPA compliant?+

Nurvivo Dental is PHIPA-aligned: Canadian data residency in AWS Canada (Toronto), signed PHIPA-aligned agreements with each practice, role-based access with SSO/MFA, encryption in transit (TLS 1.2+) and at rest (AES-256), full audit logging of every access and edit, and a zero-training policy on your clinical data.

How is patient consent for ambient capture handled?+

Consent is captured at intake using plain-language wording your practice can adapt, recorded per patient, and revocable at any time. Revocation halts new ambient capture immediately for that patient. Every consent event is written to the audit log.

How long are notes and audit logs retained?+

Signed text notes follow your practice's standard dental record retention (typically 10 years past last visit per provincial college guidance). Raw audio is never retained. Audit logs are retained for the same period as the chart they describe.

Does the AI train on my patient conversations?+

No. Nurvivo Dental operates a zero-training policy — your clinical conversations and notes are never used to train external or internal AI models without an explicit, separate written opt-in.

Can I customize the note template?+

Yes. Nurvivo Dental supports SOAP, DARE, narrative, and fully custom templates per provider or per procedure type.

How does it fit into a normal operatory workflow?+

The dentist works as normal. The scribe runs in the background, the draft appears in the chart at the end of the appointment, the clinician reviews and signs in under a minute, and confirmed codes flow to the treatment plan and insurance pre-auth automatically.

What happens if the dentist disagrees with the draft?+

Every field is editable. The dentist is always the final signer and the source of truth — the AI draft is a starting point, not a final document.

Does it integrate with my practice management software?+

Nurvivo Dental supports common PMS workflows and is expanding native integrations through 2026. Where a native integration isn't yet available, notes and codes can be copied or exported into the existing chart with minimal friction.
